When a client asks why a cheaper quote was passed over, or an E&O reviewer asks who approved a limit above authority, the answer should not be “check the email”. The decision record holds it.
What is written, and when
- Every Analyze writes a run: the placement controls evaluated on the deal, each hit with its rule, version and reason, and a verdict. The previous run stays; the new one is current.
- Every answer to a prepared decision writes a decision: what was prepared, the answer, the note, what was carried out, and a human sign-off.
- Every sign-off by a practice leader is signed onto the run that referred it.
| Section | Hit | Outcome |
|---|---|---|
| Placement control | BIND_ABOVE_AUTHORITY — the limit is above the deal broker's authority | refer |
| Placement control | RETENTION_OUT_OF_BAND — retention outside 0.5%–1% of EV | warn |
| Prepared decision | Accept: Select Meridian Transaction Re (illustrative)'s quote | proceed |
| Prepared decision | Reject: Approach 4 markets — client prefers two | decline |

The highest-ranked outcome present wins. The agents never set the verdict; it is the controls’. The supervisor agent reads it and prepares the brief.
